A new way to look at patient journeys

ICS are now in place and according to the Kings Fund have four key aims:

  • Improving outcomes in population health and health care.
  • Tackling inequalities in outcomes, experience and access.
  • Enhancing productivity and value for money.
  • Helping the NHS to support broader social and economic development.

And they highlight that this will involve collaboration across providers in order to improve outcomes and deliver population based health.

 

Delivering on these big aims will require courage, innovation, and collaboration.

Naturally organisations look at problems in silos, why? Because it’s safe, because it’s much easier, because that’s not what we are measured on, because it’s not within our scope and our assumptions about what enters our realm are unchallenged and the implications of what leaves, well that’s not our problem.

 

What if we did things differently, looked at this in a different way, changed the way we collaborate with other organisations?
